（1）資格要求 Requirements for Qualification of Bidder A．★營業(yè)執(zhí)照：如果投標(biāo)人為境內(nèi)注冊(cè)公司，投標(biāo)人需提供合法有效的企業(yè)法人營業(yè)執(zhí)照、稅務(wù)登記證及組織機(jī)構(gòu)代碼證或證照合一的營業(yè)執(zhí)照；如果投標(biāo)人為境外注冊(cè)公司，需提供有效的公司登記注冊(cè)證明；A．★Business licence：If the bidder is a domestic registered company, the bidder is required to provide a legally valid enterprise legal person business license, tax registration certificate and organization code certificate or business license with one certificate; If the bidder is an overseas registered company, the bidder is required to provide a valid certificate of company registration;B、★制造商投標(biāo)要求（不接受代理）：B、★Bid by Manufacturer requirement （Bid by Agent is not accepted）投標(biāo)人應(yīng)為所投貨物的制造商。本次招標(biāo)不接受代理商投標(biāo)。The bidder shall be the manufacturer of bid goods. Failure to obtain bidding documents through legal channels：Participation in bidding is not allowed..（2）★業(yè)績要求Requirement for Bidder's reference：自2005年1月1日至投標(biāo)截止日（以合同簽署時(shí)間為準(zhǔn)），投標(biāo)人應(yīng)具有至少1個(gè)合同的永久多點(diǎn)系泊錨腿張緊系統(tǒng)的供貨業(yè)績（投標(biāo)人必須是供貨業(yè)績的制造商），且滿足：1) 錨機(jī)為立式液壓驅(qū)動(dòng)連續(xù)旋轉(zhuǎn)鏈輪式錨機(jī)，且適用于系泊錨腿鏈的最小破斷力不小于24281kN；2) 水上止鏈器適用于系泊錨腿鏈的最小破斷力不小于24281kN；3) 水下七槽旗型鏈輪導(dǎo)鏈器適用于系泊錨腿鏈的最小破斷力不小于24281kN；如投標(biāo)人提供的永久多點(diǎn)系泊錨機(jī)系統(tǒng)單個(gè)業(yè)績合同無法同時(shí)滿足1)、2)和 3)，接受多個(gè)業(yè)績合同累計(jì)滿足1)、2)和 3)要求。投標(biāo)人須按規(guī)定格式提交業(yè)績表，并提交相關(guān)業(yè)績證明文件。業(yè)績證明文件包括：1）銷售合同和2）設(shè)備總布置圖及關(guān)鍵參數(shù)和3）用戶簽字或蓋章的到貨驗(yàn)收材料（到貨驗(yàn)收單或調(diào)試文件或設(shè)備接受證明文件或完成系泊回接及張緊的完工證書或產(chǎn)品船級(jí)社證書）。投標(biāo)人所提交的業(yè)績證明文件必須至少體現(xiàn)以下內(nèi)容：合同簽署時(shí)間、合同簽署頁（國內(nèi)貿(mào)易合同應(yīng)有雙方蓋章，國際貿(mào)易合同應(yīng)有雙方簽字或蓋章）、貨物名稱、供貨范圍、上述1）、2）、3）的技術(shù)要求、用戶簽字或蓋章的到貨驗(yàn)收材料。若業(yè)績合同為年度協(xié)議，除提供年度協(xié)議外，還應(yīng)提供相應(yīng)的已到貨訂單，訂單內(nèi)容或編號(hào)應(yīng)與年度協(xié)議相關(guān)聯(lián)。同一個(gè)年度協(xié)議下提供1個(gè)或以上的訂單及與訂單對(duì)應(yīng)的到貨驗(yàn)收材料均算為1個(gè)有效業(yè)績。未提交業(yè)績證明文件，或通過所提供的業(yè)績證明文件無法認(rèn)定滿足上述業(yè)績要求的，均視為無效業(yè)績。The manufacturer shall has delivered (the bidder shall be the manufacturer of reference project delivered)at least one set permanent multi-point mooring leg pulling system from 1st of Jan 2005 to bidding data(based on contract date), technical requirement are:1)The vertical continuous rotation hydraulic driven windlass shall be designed for the mooring leg chain that MBL is not less than 24281kN;2)The above water chain stopper shall be designed for the mooring leg chain that MBL is not less than 24281kN;3)Underwater fairlead (seven whelp pockets flag chain sheave) shall be designed for the mooring leg chain that MBL is not less than 24281kN.If the bidder’s single project reference can not meet with above requirements 1),2) and 3), several different projects reference shall be provided to meet with above requirements 1), 2) and 3) individually.The bidder shall submit reference document as reuired format, including 1) Copies of sales contract/PO, 2) General layout drawing and key technical parameter,3) Delivery acceptance materials signed/stamped by end user (delivery acceptance form or commissioning report or equipment acceptance certificate or completion certificates that mooring leg hook up& tensioning completed or products classification certificate).The reference documentary submitted by the Bidder shall include the information at least as follows: contract signing time, contract signing page (domestic trade contracts shall have seals from both parties, and international trade contracts shall have signatures or seals from both parties), equipment name,supply scope, above technical requirement 1) and 2) and 3) delivery acceptance materials signed/stamped by end user or products classification certificate.If the contract is an annual agreement, not only the annual agreement shall be provided, corresponding orders, which has been delivery successfully shall also be provided. And the order content or number shall be associated with the annual agreement. Providing one or more orders and corresponding delivery acceptance materials under the same annual agreement is considered ONE valid trace record.Absence of the reference evidence documents or failure to verify the foregoing information shall be deemed as an invalid track records.
